HealthCheckPlus was designed to facilitating real-time health screening and knowledge mobilization in a primary care setting in a manner that adds no additional burden to physicians' time.
How the system works:
The system is implemented on a secure touch screen kiosk that can be positioned in either the clinic waiting room or in examination rooms as desired.
Patients complete screening tool in the physician's office in about the 5 to 10 minutes. The system immediately generates and prints a 1-page report on a dedicated or networked computer, upon completion of the screening survey. The report highlights elevated symptom scores, risk factors, and patient identified needs. Alternately, the system can be integrated with an existing electronic chart system. |

Additional Features:
There are several additional features of the system:
Assess Patient Care Experiences. Health Check Plus can be used evaluate a range of patient care experiences (e.g., access, quality of care, stigma, etc.)
Mobilize Knowledge. Remain up to date on treatment guidelines for a range of difficulties. Disseminate patient resource pages automatically depending on identified clinical needs.
Multi-lingual Capacity. The system is currently available in six language including, English, French, Spanish, Arabic, and Chinese.
Quarterly Summary Reports: Quarterly reports summarizing the difficulties of patients reported in your clinic can be easily generated by age, gender, or treating physician as required.
Facilitate Documentation and Self Study: Health Checkplus can be used to assist physicians in conducting a self-study of their practice and clinic.
Flexible: Both systems utilize flexible infrastructure that can be programmed to implement any number of screening modules, add new modules, modify existing modules, or target specific age or demographic groups.
Multiplatform: The screening tools can also be implemented on a touch screen kiosk, as well as on a desktop computer, PDA or smart phone. |
In addition to routine screening, HealthCheckPlus offers the built in capacity to assess patient care, improve the utilization and familiarity with treatment guidelines, as well as monitor clinic needs and enhance clinical decision making. Clinical practice guidelines, advisories and warnings, information sheets, or self-management recommendations for patients, can be printed on-demand or keyed to the symptom profile identified through screening.
