SymptomTracker was designed to facilitate assessing patient outcomes in real-time providing clinicians with concise reports, summarizing progress, baseline, test scores, as well as adverse responses to treatment if desired.
How SymptomTracker Works:
Clinicians are provided with access cards which are distributed to clients as they begin a treatment program. Upon logging on, the system generates a unique account for each individual. A valid e-mail account is optional but if entered enables the system to send weekly reminders to patients to complete measures. Accordingly, no personal information is required.
The system is extremely flexible and can be configured to administer any number of questionnaires, in different languages, and at different time frames (e.g., weekly, daily, monthly, or after the fourth login), irrespective of the respondent (e.g., parent, child or professional). |

Additional Features:
Customizable. This system currently screens for a variety of domains, but can be configured to assess any number or combination of domains, using existing or custom-built screening tools. Current modules include: Mobility & Exercise, Sleep, Depression, Anxiety, Self Harm, Suicide, Alcohol Use, Drug Use, Gambling, Smoking, Aggression towards others, Bullying & Harassment in the Workplace, Eating Disorders, Sexually transmitted Infections, Pain).
Assessing Patient Care Experiences. The system can be used evaluate a range of patient care experiences (e.g., access, quality of care, stigma, etc.) either on-site or post-visit through a secure link to a follow-up questionnaire.
Multi-lingual capacity. Meeting the health care needs of new non-English speaking Canadians may be difficult. The tools are currently offered in English and French, but can be administered in any language including Arabic.
Quarterly Summary Reports. Quarterly reports summarizing the difficulties of patients reported in your clinic can be easily generated by age, gender, or treating physician as required.
Facilitate Self Study The system can be used to assist physicians in conducting a self-study of their practice and clinic.
Flexible. The system has a flexible infrastructure that can be programmed to implement any number of screening modules, add new modules, modify existing modules, or target specific age or demographic groups.
Multiplatform: Questionnaires and checklists can be implemented over the internet, PDAs or smart phones. |
